Save the contact card so you'll recognize the call when it comes — and so it doesn't look like spam. You can reply STOP at any time to opt out.

A call with Rosemary, about 20–30 minutes.
Rosemary is a digital assistant on the phone — calm, patient, and built for this kind of conversation. She'll talk through what matters to you and the kinds of treatment you would or wouldn't want, then put it into an advance care planning form. Depending on what's right for you, this can happen in one call or over a couple of shorter ones — and you can pause, skip a question, or pick a better time at any point.

3The same day it's signed
You get a copy — and so does your chart.
The signed form is filed directly into your medical record, and a printed copy is ready for you to pick up at Dr. Aaron's office at your next visit — for you, or for any family member or healthcare proxy you'd like to share it with.

Things patients often ask.
Is this a sales call?
No. Rosemary is part of Dr. Aaron's care, not a salesperson. We won't ask you to buy anything, sign up for anything, or share financial information.
Is there any cost to me?
No. Rosemary is covered through Dr. Aaron's practice. You will not get a bill, and this is not billed to Medicare in a way that costs you anything out of pocket.
Who will see what I share?
Dr. Aaron and the clinicians who already see your medical record. Your conversation becomes a clinical note in your chart, protected by the same HIPAA privacy rules as the rest of your care.
Am I required to do this?
No. It is completely voluntary. You can decline the call, end it at any time, or tell us not to contact you again. Your care with Dr. Aaron will not change either way.
Is Rosemary a person or a computer?
Rosemary is a digital assistant designed for this kind of careful, patient conversation — she leads the call and helps you put your wishes into writing right then. If you'd prefer to talk to a person instead, just say so and we'll connect you.
Do I have to sign anything?
Only if you want to. Putting your wishes into a signed form makes them easier for your care team to honor, but it's your choice. You can take as much time as you need on the call, and Rosemary will walk through any part of it as many times as you'd like.
What if I don't want a call right now?
Reply STOP to the text we send you, or call Dr. Aaron's office and let them know. We'll stop reaching out.
